Training in Addiction Psychiatry

There are two levels of training available in addiction psychiatry. The more fundamental of the two levels is a two-month rotation for residents in general psychiatry, which is taken in the second, third or fourth year of that program. The more advanced training is a year of subspecialty training, taken in the fifth post-graduate year, and is designed to prepare the resident for the examination in addiction psychiatry. Both programs are offered in affiliation with the University of Louisville Department of Psychiatry and Behavioral Sciences.

Rotation for General Psychiatry Residents:

General psychiatry residents spend 1 or 2 months of their 2nd, 3rd or 4th years of training with Dr. Arthur Meyer, as determined by the general psychiatry training director. The residents will have the opportunity to be trained in the evaluation and treatment of chemically dependent patients most of which also have other psychiatric disorders. The residents will be involved in all aspects of the overall management of these patients, focusing on the initial evaluation, the ongoing treatment throughout the patient's stay in the hospital, including all modalities of treatment utilized, and discharge planning. Experience will be gained in medical detoxification and the use and misuse of psychiatric medications in this population. The residents will be expected to devote full-time to their experience with Dr. Meyer, except for the required didactic training. The caseload will consist of 6 to 8 patients, depending on the residents' ability to develop the necessary skills. There will be no after hours call.

Subspecialty Training in Addiction Psychiatry:

The PGY-5 training will consist of one year of advanced training in addiction psychiatry, which will enable those completing the program to sit for the examination in Addiction Psychiatry. Applicants must be eligible for a license to practice medicine in Kentucky and that license must be in place to begin the training program. The program will provide a broad-based spectrum of sites and treatment experience, enabling addiction residents to become proficient in:

" Substance abuse disorders, their diagnosis and differential diagnosis, symptoms of intoxication, withdrawal, overdose and its management.

" The epidemiology and prevention of substance use disorders as influenced by social, cultural, genetic, economic and pharmacological factors, including special populations.

" Therapeutic modalities commonly used to treat substance use disorders, group, couples, family, insight oriented and cognitive therapies, as well as 12-step facilitation and pharmacotherapy.

" Current research interest in addiction psychiatry and the conceptualization of a research project.

" Conducting a comprehensive evaluation of the addicted patient and creating appropriate documentation.

" Developing a comprehensive assessment and formulating a comprehensive treatment plan to encompass the management of detoxification, treatment of concomitant psychiatric disorders, management of medical problems and identifying relevant psychosocial issues.

" Supervising junior residents and medical students in the assessment and management of addicted patients.

" Developing and implementing a research project that is submitted for publication.

" Evaluating the patient's financial resources and appropriately matching the patient to the services available in the community.

At the end of this one-year program, the addiction resident's behavior will reflect:

" Consistent understanding and compassion for the addicted person.

" Awareness of the indications, benefits and limitations of all currently accepted treatment modalities and of the addiction psychiatrist.

" Respect for the contribution of other health care professionals participating in the care of the addicted patient.
